I dunno about Spain, but the STD thing isn't much of a problem among the licensed prostitutes/escorts in Germany, it's legal, and they're careful and have regular checkups, so I'd say have fun with the girl and let the old perv watch and jack off. I don't think this answers her question or is helpful in any way. Are we really as judgmental as vanilla people? - - Yes, unfortunately some swingers are as judgemental or even worse than our vanilla counterparts.
